It's important to know exactly which services will and will not be covered by your insurance plan. Knowing your deductible, out of pocket, copay and coinsurance will help you better determine your financial responsibility for each appointment.

It's simple! Begin by calling the member services customer service number on the back of your card.

Let them know you would like an explanation of your financial responsibility for behavioral health benefits for our specific practice - Alpine Psychiatry. You will have to provide them with some numbers before they can give you an accurate explanation. This is what you will provide to them:

1. NPI & Tax ID - for Alpine Psychiatry - NPI: 1134789662 & Tax ID: 84-2041009
2. Tell them for the first evaluations (In-office), we use code 99205 & bill $463.78. OR 90792 & we bill $400.00
3. Tell them for all telehealth and in person follow up appointments we use code 99214 & bill $230.00. 4. Ask them to explain your deductible & out of pocket and how they will determine when and how much you will have to pay before your insurance pays.
5. Know exactly what you will be financially responsible for regarding your copay and coinsurance for each visit before you meet both your deductible and out of pocket.
